Simply complete this form and take it to your GP to sign. Then take it to your case manager (if you qualify) and they will grant you $61.69 each week as a contribution. WINZ grant up to 10 sessions. In some cases, WINZ allow fees to be paid directly into my bank account. However, this is on a case-by-case basis.

All WINZ payments, MUST be paid upfront prior to each counselling session. It takes approximately 2 - 3 weeks for WINZ to approve payments into your bank account. No appointments will be scheduled or proceeded without payment. My fees are $75 - 110 per WINZ client, which means you will have to contribute the balance and transfer WINZ payments into my bank account prior to each session or on the day.
